01. Fatigue that doesn't improve with sleep
You sleep eight hours and wake up tired. This is one of the most consistent signs of hormone dysregulation, and it usually points to one of three things: low cortisol (your body's morning wake-up signal), low thyroid hormone (which controls the speed of your metabolism at the cellular level), or low testosterone — in both men and women. When any of these are off, rest doesn't restore you the way it should.
02. Brain fog or difficulty concentrating
Estrogen has direct effects on the brain — it supports serotonin and acetylcholine production, two neurotransmitters involved in memory and focus. When estrogen drops, cognitive sharpness often goes with it. Thyroid dysfunction does the same thing: low T3 slows neural signaling, making it harder to retrieve information and think clearly. This is not a mental health issue. It's a biochemical one.
03. Unexplained weight gain — especially around the midsection
Abdominal fat is closely tied to cortisol and insulin. When cortisol is chronically elevated (from stress, poor sleep, or adrenal dysfunction), it drives fat storage around the organs. At the same time, low estrogen shifts where the body stores fat — from the hips and thighs toward the abdomen. Low thyroid compounds this by slowing how many calories you burn at rest. It's rarely one factor alone.
04. Difficulty losing weight despite doing everything right
This one is different from gaining weight — it's the frustration of eating well and exercising consistently with no result. The most common culprit is thyroid underfunction, specifically low free T3, which is the active form of thyroid hormone that your cells actually use. Standard labs often only test TSH, which can look normal even when T3 is insufficient. Insulin resistance — which estrogen helps prevent — also blocks fat burning regardless of caloric intake.
05. Low libido
Testosterone is the primary driver of libido in both sexes. In women, it's produced in smaller amounts but plays a significant role in sexual desire, arousal, and satisfaction. Estrogen also matters — it supports lubrication and sensitivity. When both decline, as they often do together during perimenopause, libido drops. In men, testosterone decline is the main driver, but low thyroid and elevated cortisol also suppress sexual interest.
06. Mood changes, irritability, or low-grade anxiety
Estrogen modulates serotonin receptors. As estrogen fluctuates — especially in perimenopause — mood becomes less stable. Progesterone is the calming hormone; when it drops faster than estrogen, the imbalance between the two creates anxiety, irritability, and a shorter fuse. This is sometimes called estrogen dominance, even when total estrogen isn't high — it's the ratio that matters.
07. Sleep disruption or waking between 2–4 AM
Progesterone has a sedative effect and helps initiate and maintain deep sleep. When it drops, sleep becomes lighter and more fragmented. Night sweats — caused by estrogen fluctuation triggering the brain's thermoregulation — wake people in the early morning hours, often right on schedule. Cortisol also follows a circadian rhythm; if it rises too early or spikes at night, it pulls you out of sleep.
08. Hair thinning
Hair follicles are sensitive to hormonal shifts. In women, low estrogen and low thyroid are the two most common causes of diffuse thinning — meaning hair thins across the scalp rather than in patches. An excess of androgens can cause hair to thin at the temples. In men, testosterone converting to dihydrotestosterone (DHT) at high rates is the driver of pattern hair loss. Thyroid-related hair loss is often dismissed because it looks like normal shedding — but the volume is significantly higher.
09. Hot flashes and night sweats
These are the most recognized hormone symptoms — and they have a specific mechanism. As estrogen declines, the hypothalamus (the brain's thermostat) becomes hypersensitive to small temperature changes. It reads normal body temperature as overheating and triggers a cooling response: vasodilation, sweating, and flushing. The threshold for this response drops significantly when estrogen is low, which is why even mild warmth can trigger a flash.
10. Low motivation or a general sense of flatness
This is different from clinical depression, though it can be mistaken for it. Testosterone plays a direct role in drive, ambition, and the reward response. When testosterone is low — in men or women — the world becomes less interesting. Things that used to feel satisfying stop registering. Dopamine function is also influenced by thyroid and estrogen levels. When multiple hormones are off simultaneously, motivation is often the first thing to disappear and the last thing to come back.
What to do with this information
These symptoms don't appear in isolation. Hormones interact with each other constantly — when one shifts, others compensate or collapse. Which is why treating a single number rarely solves the problem.
